These clients receive funding through the Home and Community-Based Medicaid Waiver. Services are individualized for each person; however, some common areas of support and skill development may include:
– Residential Housing Services
– Social skills
– Community skills
– Daily living skills
– Safety skills
– Shared Living Programs
– Host Home Program
CentrePoint Support Living’s residential program offers individualized living arrangements for individuals who can benefit from living in a private home with a family or individual. Individuals enjoy living in a family setting, engaging in community activities, and sharing household responsibilities. Our host home providers create a caring support system and assist the individual with personal care, meals, finances, medical appointments, and community connections. Providers view the positive capabilities of the individuals in their home and help the person have a normal a life as possible.
